The difference

Most practices are built around the clock. Dr. Kakodkar’s is built around you.

Insurance pays a practice for the number of people seen, not for the care given. That math is why visits are short and why a routine appointment is three weeks out. Membership removes the math. You pay the practice directly, the patient list stays small on purpose, and your visit lasts as long as your health needs it to.

Where you are now

  • Seven to fifteen minutes, and the clock is in the room
  • A copay every visit, and a bill weeks later you cannot read
  • Three weeks for a routine appointment
  • Your message goes to a portal, and someone answers eventually
  • Two to three thousand patients on one doctor's list

Here at Connected Care MD

  • Thirty to sixty minutes, and nobody is watching the clock
  • One monthly membership, and nothing charged per visit
  • Same day or next day, most of the time
  • You text or call Dr. Kakodkar, and Dr. Kakodkar answers
  • A small list, so she knows your history instead of reading it back

Membership is not insurance and does not replace it. Most members keep a plan for the hospital, surgery, specialists, and emergencies. See what to pair it with

Dr. Swati Kakodkar, family physician at Connected Care MD in Hillsboro, Oregon
Your doctor

Swati Kakodkar, MD, DABOM

I trained in gynecology in India and later completed my family medicine residency at the University of Arkansas for Medical Sciences. I am also board-certified in obesity medicine, with a strong focus on preventive health, weight and metabolic health, and long-term wellness.

Over the past twenty years, I have learned that sleep, weight, mood, hormones, and everyday health are often connected. Good medicine starts with listening, looking at the whole picture, and getting to know the person behind the symptoms.

That is why I built my practice around relationships, prevention, and continuity of care. My goal is to be a physician who truly knows you, understands the bigger picture of your health, and is accessible when you need care.

Outside of medicine, I enjoy traveling, cooking, and creative crafts.

Where we spend the most time

If you’ve been told it’s just part of getting older, come sit down. Let’s talk.

Sleep that isn’t what it used to be. Weight that feels harder to manage. Changes in mood. Periods that have become different or unpredictable. These concerns can be connected, and they deserve more than a rushed conversation.

With my background in gynecology and board certification in obesity medicine, I take a broader view of your health. We can look at hormones, weight and metabolic health, sleep, mood, and your overall wellbeing together, rather than treating each concern in isolation.

Your membership includes visits and evaluation for these concerns. When hormone therapy is appropriate for you, prescribing and managing that treatment is also included, with no separate fee from the practice.

Pellet therapy is not currently offered.

Medical weight management

Weight is about more than willpower. We treat it that way.

Weight can be influenced by many things, including sleep, hormones, muscle mass, stress, insulin resistance, thyroid health, menopause, and medications. Understanding what may be contributing to your weight is an important part of medical care.

As a physician board-certified in both obesity medicine and family medicine, I can address your weight and metabolic health alongside the rest of your health, all in one place.

Your weight-management care may include:

  • Body composition, not just the number on the scale
  • Metabolic evaluation, including insulin resistance, prediabetes, thyroid health, and cholesterol
  • Practical nutrition and strength guidance designed to be sustainable
  • Medication when medically appropriate, with ongoing prescribing and monitoring
  • Regular progress reviews, with adjustments when something isn’t working
  • Your overall health considered alongside your weight, rather than treating the two separately

For members, our dedicated weight-management program is $29 per month in addition to your membership. Your routine primary care for conditions such as blood pressure, diabetes, thyroid health, and cholesterol is already part of your membership, with no additional practice fee.

For patients who are not local, weight-management care is also available by telemedicine, with pricing outlined below.

About the medications. When a weight medication is right for you, we prescribe it and we monitor it. The prescription goes to whichever pharmacy you choose, including mail order. You pay the pharmacy directly. Medication costs are never part of your membership or of any consultation fee; they vary by pharmacy, dose and supply, and they can change without notice. We will tell you what to expect before you leave with the prescription.

Couples7.5% off

When two of you enroll together, whichever tiers you each fall into.

These two do not stack. If you qualify for both, your saving is 7.5%.

Every membership here is $150 a month or less, and that is on purpose. It keeps your membership inside the health savings account limit, so you can pay for it with pre-tax dollars. The limit is $150 a month for an individual and $300 for a family. Your plan administrator can confirm your own situation.

And Oregon changed the rules in your favor on 1 January 2026. Under House Bill 2540, most Oregon health insurers now have to count qualifying out-of-pocket payments toward your annual deductible, even when the care was not in your network. The care has to be medically necessary, covered by your plan, and cost less than the same care would have through your insurance. Kaiser Permanente plans are not included. Bring your plan to your meet and greet and we will work out together whether yours qualifies.

Materials for a procedure are billed at our cost and priced before we start, so you always know what a thing costs before it happens.

A note about pain management. We do not prescribe opioids for chronic pain management. If opioid treatment is needed, that part of your care will need to be managed by an appropriate pain specialist. We will continue to care for your overall health and can help coordinate your care with the right specialist when needed.

Already have insurance?

Membership is not insurance. It works alongside it.

Your membership is designed to cover the care you need most often: everyday visits, questions, follow-ups, and ongoing primary care.

It does not replace health insurance. We strongly encourage members to maintain appropriate health insurance coverage for unexpected or higher-cost medical needs, such as emergencies, hospital stays, surgery, specialist care, and other services not included in the membership.

Think of your membership as your home for everyday care, with insurance there to help protect you when bigger medical needs arise.

Keep what you have

Your employer's plan

There’s no need to change your employer health plan. Keep it for hospital care, specialists, and the unexpected. Use your membership for everyday primary care and direct access to your doctor when a visit isn’t necessary.

What most members choose

A high-deductible plan with an HSA

A high deductible brings the premium down, and the plan still catches you if something serious happens. Because every membership here is $150 a month or less, you can pay for it out of the HSA with pre-tax dollars.

65 and over

Medicare

If you are over 65 and not on Medicare, you can join today at $149 a month. If you have Original Medicare, with or without a Medigap supplement, there is no membership fee for you: We take a small number of Medicare patients and bill Medicare in the usual way, so your cover carries on exactly as it does now, for your primary care as well as for hospitals and specialists. We do not take Medicare Advantage plans. If you have one, come and talk to us anyway and we will work out together whether this can work for you.

What membership does not cover

  • Hospital stays and surgery
  • Emergency room care
  • Specialist visits and consultations
  • Advanced imaging such as MRI and CT
  • Prescriptions filled outside our dispensary
  • Maternity and obstetric care

This is why you keep coverage.

Membership covers your everyday care. It does not replace insurance for the big, unexpected things. If you are not sure how your plan fits alongside it, bring it to your meet and greet and we will look at it together.

Not near Hillsboro?

Menopause & Weight Care by Telemedicine

Dr. Kakodkar offers care by telehealth to patients in Oregon, Washington, California, and Texas. Meet with her by video from wherever you are, with no drive to Hillsboro required.

This is not our local membership practice, and it is not a website where you fill out a questionnaire and receive a prescription from someone you’ve never met.

This is dedicated care for the two things women often wait the longest to get real help with: menopause and weight. You’ll see the same doctor at every visit and have the time needed to understand what’s going on and decide what to do about it.

Menopause & Midlife

Hormones, sleep, mood, and everything that comes with this stage of life

Your first visit is a full hour devoted to your symptoms, medical history, and goals. We’ll talk about what you’re experiencing, which labs are worth running, and whether hormone therapy is appropriate for you.

You’ll leave with a plan, not another referral.

Initial visit, 60 minutes, labs included$299
Single follow-up visit$120
Ongoing care$79/month
Weight & Metabolic Health

Understanding your weight, your metabolism, and what’s driving them

Weight gain is rarely as simple as eating less and moving more. We start with your history, your current health, and a metabolic evaluation to identify what is actually contributing to the problem.

The goal is to build a treatment plan based on what we find, not simply to chase a number on the scale.

When appropriate, medications can be prescribed and monitored as part of that plan. Medication costs are separate and paid directly to your pharmacy.

Initial visit, 60 minutes, labs included$299
Single follow-up visit$120
Ongoing care$79/month

What’s Included

The initial visit includes the full hour, your treatment plan, and basic annual laboratory testing.

If additional testing is needed, you’ll know the cost before anything is ordered. Anything beyond the basics is billed at our discounted cost rather than typical retail pricing.

The monthly care plan includes follow-up visits, messaging between visits, prescription management, and ongoing monitoring. There are no additional physician fees beyond the monthly charge.

The $120 follow-up visit is available for patients who want a one-time follow-up without ongoing care.

Medication costs are always paid separately to your pharmacy.

What Telemedicine Can and Cannot Do

Video visits are ideal for menopause care, hormone therapy, weight management, metabolic health, and the longer conversations these issues deserve.

They are not a substitute for an examination, a procedure, or emergency care. You’ll still need a local physician for problems that require hands-on evaluation, and we will tell you plainly when something needs to be addressed in person.

Prescribing follows the laws of the state where you are physically located during the visit, and some medications cannot be prescribed through telemedicine.

Questions

The things people ask before they join.

Is this insurance?
No. Your membership pays for primary care directly, with no claim filed. Keep a plan for the hospital, surgery, specialists, and emergencies. Membership covers the everyday care in between.
I already have insurance. Does this replace it?
No, and you should keep it. We do not bill your insurance for membership visits, so there are no claims, no copays, and no statements arriving three months later. Many members pair a high deductible plan with a membership here.
What if I have no insurance at all?
Your everyday care is covered by your membership, and you pay nothing per visit. For the larger things it does not cover, hospital care, surgery, specialists and emergencies, you still need a plan. Bring what you are considering to your meet and greet and we will look at it together.
Do you manage chronic pain?
Yes, with one limit: we do not prescribe opioids for pain management. If your treatment needs opioids, a pain specialist handles that part of your care and we look after everything else alongside them. We would rather say so plainly now than have you join and find out later.
Do you prescribe ADHD medication?
Not as a general rule. ADHD medication is something we decide case by case rather than something this practice prescribes routinely, and in most cases we will send you to a psychiatrist for the diagnosis and the prescribing. If you are already settled on a treatment that is working, bring it to your meet and greet and we will talk it through. Everything else about your mental health, including anxiety, depression and sleep, is part of your membership.
What do labs, imaging and medications cost?
Your annual preventive labs are part of your membership: complete blood count, comprehensive metabolic panel, A1c, TSH and a cholesterol profile. Anything beyond those is billed at our discounted rate and priced before you agree to it. Imaging is not included in your membership: when you need an X-ray, an ultrasound, an MRI or a CT, we arrange it and you pay for it yourself at the discounted self-pay rate, which is usually well below what a hospital would bill you, and you will know the price before anything is booked. Medications we keep in stock are dispensed here at our cost, which is often less than a pharmacy would charge you. No markup anywhere, and no surprise.
Is menopause and hormone care included?
Yes. The visits, the evaluation, the ongoing management of perimenopause and menopause, and the hormones we prescribe for you are part of your membership, with no separate fee. Your well-woman exam is included too. To be clear about one thing people often ask: this is prescribed hormone therapy, and we do not offer pellet therapy yet. Labs are at our cost, and the medication itself is billed by your pharmacy like any other prescription.
What is the weight management program?

The Weight Management Program is an optional add-on to the regular membership, designed for people who want more focused support with weight and body composition.

  • Cost: $29/month in addition to the membership fee
  • Body composition analysis: Looks beyond the number on the scale to provide a fuller picture of progress
  • Nutrition coaching: Personalized guidance to support sustainable nutrition and weight goals
  • Medical weight management: Care from a physician who is board-certified in obesity medicine
  • Medications: If prescribed, medications are billed separately

Importantly, you don’t need this add-on for routine metabolic healthcare. Management of blood pressure, diabetes, thyroid conditions, cholesterol, and other metabolic health concerns is already included with the standard membership.

In short: the $29 add-on is for specialized weight-management services, not for accessing routine metabolic care.

Is there a joining fee?
Yes, $75 once, when you enroll. After that it is your monthly membership and nothing charged per visit. If you leave and later rejoin, the enrollment fee is charged again, because the work of picking your history back up starts over.
How do I pay?
By card or by bank transfer, whichever suits you. Your membership is charged automatically on the first of each month, so there is no invoice to chase and nothing to remember. If you join partway through a month, your first payment covers only the days until the first, so everyone is on the same cycle after that. You can change the card on file at any time. Your card details are held by the billing service, not by us, so we never see your card number. If a payment fails you will hear from us first; cards expire, and that is not a reason to lose your doctor.
Can I pay with my HSA?
Every membership here is $150 a month or less, which keeps it inside the health savings account limit of $150 a month for an individual and $300 for a family. That means you can generally pay with pre-tax dollars. Your plan administrator can confirm your own situation.
I am 65 or over. Can I join?
If you are not on Medicare, yes, today, at $149 a month. If you have Original Medicare, with or without a Medigap supplement, you do not pay a membership fee at all: We take a small number of Medicare patients and bill Medicare in the usual way, so nothing about your cover changes. We do not take Medicare Advantage plans, and if that is you, come and talk to me anyway rather than assume the answer is no. Space for Medicare patients is limited either way, so come early.
I am on the Oregon Health Plan. Can I join?
No. Membership is not available to patients enrolled in the Oregon Health Plan (OHP), whose primary care is covered at no cost through their plan, and Oregon's rules do not allow us to charge OHP members for care the plan covers. Your plan will assign you a primary care provider in its network, and we are happy to help you find one. If your coverage changes later, we would be glad to talk.
What if I need a specialist or the hospital?
We refer you and we stay involved. That is what insurance is for, and it is why we recommend keeping a plan alongside your membership.
Can I cancel?
Yes. After your initial three-month commitment, you can cancel at any time, for any reason. The first three months give us time to thoroughly understand your health history, establish your care, and begin putting that work into action. Your $75 enrollment fee is nonrefundable, and if you decide to rejoin in the future, a new enrollment fee will apply.
Can I be seen without joining?

Yes. If you’re not a member and don’t have insurance coverage, you can still be seen on a self-pay basis. An acute visit is $199.

An annual physical includes a full one-hour visit, preventive labs, and a follow-up visit to review your results. The self-pay price is based on age:

  • Ages 18–29: $237
  • Ages 30–44: $297
  • Ages 45–64: $357
  • Ages 65+: $447

For patients without insurance coverage, the annual physical package is priced at three times the monthly membership rate for your age. This keeps our self-pay pricing straightforward while also making it easy to compare the cost of a one-time physical with ongoing membership.

Children are seen through membership when a parent is enrolled rather than on a one-time basis. All non-member services are paid at the time of the visit.

If you decide afterward that you’d like to become a member, we’re happy to discuss it, but there’s no obligation or pressure to join.
